That was real. The springboard's mechanism to prevent exactly that from happening failed. In the video, the announcer exclaims "I have never seen that happen before!"

This is fake, but it could happen. The springs in those spring boards are hold down extremely tight, if the spring board would break badly enough, this would happen. I've never heard of a real accident though.
